Personally Yours has been providing patient advocacy, care management/ care coordination/ household management and concierge services throughout the South Jersey area for the past 20 years to meet a need in the community for an ethical, client-centered, proactive, holistic approach to aging with dignity. During this time, our mission has stayed consistent in providing peace of mind for the families and loved ones of our clients, by this we act as an extension of their family in order to assist with lifestyle management.
Within the last several years, our services have expanded into patient advocacy services along with care coordination due to the changing needs of our clients. Without a support system in place and without an advocate to assist them, more and more clients are coming to us with their vast needs, requiring a holistic approach to lifestyle management. More recently, Personally Yours has been providing consistent well-being visits for clients. And with the information obtained through a cohesive effort, we were able to drive care provision through our team approach to maximize positive outcomes as well as meet the expectations and ever-changing needs of our clients and their families.
